4/5/2024 EnchantaVaughn Approved QMHPA arrived to the home of Ms. Broadnax. Upon arrival at Ms. Broadnaxâs residence, QMHPA was greeted by Ms. Broadnax who appeared to be frustrated and irritated evident by her facial expression. She was alert and oriented, but not completely cooperative with QMHPA. Upon arrival, she did not present any symptoms of S/I. QMHPA began the session by assessing Ms. Broadnaxâs emotional well-being. QMHPA continued to reinforce the importance of identifying the triggers of her stress, depression and anxiety. QMHPA reinforced the importance of taking her psychotropic medication as prescribed needed to stabilize her mental health and mood. QMHPA continued to encourage and promote the importance of displaying appropriate behavior while in the public setting. QMHPA taught Ms. Broadnax conflict resolution skills such as listening and understanding what is being said before reacting/responding as well as walking away from altercations. QMHPA reinforced the importance of learning to communicate/interact/socialize with other in a positive manner without becoming angry/aggressive. Ms. Broadnax appeared to be receptive to QMHPAsâ suggestions. Ms. Broadnax remained receptive to the mental health education, but displayed an unwillingness to identify her mood and demeanor, but during the session she informed QMHPA that she was stressed/depressed, frustrated and irritated with everyone. She shared that she stopped taking the medication because her girlfriend complained about her being irritable. Ms. Broadnax also shared that the medication caused problems with her sleep pattern. In addition, Ms. Broadnax shared that she has had urges to self-mutilate which is an indicator that the medication was not assisting her with stabilizing her mental health and overall mood. Ms. Broadnax remained receptive to the life skill building. She admits that she continues to have difficulty verbalizing her thoughts, feelings and emotions in a positive/effective manner to others. Ms. Broadnax also admits that she has not been successful with controlling/managing her anger/aggression or with listening and understanding what is being said before responding and reacting.
